Oh, it's made plain in the very first episode after the pilot. I don't recall the exact words, but first he addresses Fry as "young lady," then he tells him to open his other mouth. In one of the latter seasons after the show got briefly resurrected, we learn he's a great doctor when it comes to aliens, for some reason, but hopeless with humans.
